Requirements:
· Signed Broker‐Carrier Agreement ‐ make sure to sign and fill out completely
· Completed W‐9
· Completed Direct Deposit Form
· Worker’s Compensation Insurance Copy
· Standard Rate agreement
· Copy of MC Authority Document
· Completed TSA training certificate (Training provided by VMW Express)
· All vehicles must be in safe working order with all the proper documentation.
· All drivers must have Smart phones with the capability to take clear photos and use VMW required Apps
· Certificate of Insurance --- Certificate holder listed as: VMW Express, LLC 8460-J Tyco Road Suite 61B, Vienna VA 22182
· One (1) Million Auto Liability and $100,000 Cargo Insurance is REQUIRED
